Stent and prosthesis Angioplasty, subclavian artery

54세/남자

간헐적인 좌측 손가락 저림 및 좌측 상지의 무력감으로 시행한 TFCA상 left subclavian artery의 stenosis소견이 관찰되어 stent insertion을 시행한 후 6개월에 외래에서 촬영한 F/U angiography상 intimal hyperplasia 관찰됨.

Guiding catheter를 통한 subclavian artery 자가유동류 치시진 후 시행한 혈관조형술에서 left proximal subclavian artery의 focal severe stenosis를 보였다(Figure 1). stent catheter가 stenotic portion을 지날 수 없을 정도로 stenosis가 심해 micro wire를 통과시킨 후 preballoon angioplasty를 시행하였다(Figure 2). 그리곤 exchange wire를 left brachial artery에 위치시킨 후 balloon-expandable stent를 stenotic portion에 위치시키고 deployment를 하였다(Figure 3). stent를 삽입한 후 6개월 뒤 촬영한 angiography상 stent의 직경은 그대로 유지되어 있으나 내벽의 협착진경이 가능해져 있었다. 그러나 내경의 직면 또한 stent직전에 관찰된 균등두께 보다는 stent를 삽입 후 발생한 내피 과증식에 의한 좁아짐을 알 수 있다(Figure 4).

경피적 혈관 성형술 및 스탠트 삽입술은 혈관 협착 및 폐쇄 치료에 80% 이상의 성공률을 보이는 유용한 시술이다. 그러나 시술 실패, 내피 과증식을 동반한 협착증과 스탠트 손실에 의한 동맥 손상은 일반적인 합병이다. 스탠트 삽입술 3-6개월후 재협착은 6-7%에서 나타나며 재협착의 원인으로 vessel wall recoil, intimal dissection, plaque dislodgement 및 intimal hyperplasia를 들 수 있다. 상기 환자는 30일간 clopidogrel 75mg과 aspirin 325mg 주입을 추천하였으며, 스탠트 삽입술 후 3-4.5일/주일을 주회하여 항응고제, 스탠트 삽입술 후 plavix와 aspirin을 사용한 유지 치료를 권장하였다. 총 cholesterol이 낮게 측정되어 부족한 조치를 하였으며, 활동성 ALT, AST는 증가하지 않았다.
