Iliac artery stenosis, angioplasty

67세 /남자

우측 6개월전부터 시작된 파행과 2달전부터 나타난 휴식통을 주소로 내원 함.

하부대동맥 소견상 원위부 우측 장골 동맥의 심한 협착 소견이 관찰됨. 하부 전골 후골 동맥은 잘 관찰됨 (Fig. 1). Max 9000으로 시행한 부분 혈압은 우측대퇴부가 102mmHg이고 좌측이 163mmHg으로 대퇴부 상부의 협착이 강력히 의심됨 (Fig. 2).

8mm×40mm balloon을 이용하여 확장을 시행함 (Fig.3). 풍선혈관성형술 후 시행한 조영상에서 협착부위는 사라졌음 (Fig. 4). 추적 검사에서 우측 대퇴부 압력이 154mmHg로 증가되었고 ABI가 0.52에서 0.86으로 호전되었음. 풍선확장시 정맥으로 3000 unit Heparin을 투여후에 8mm×40mm 크기의 Balloon으로 2분간 확장을 지속시킨 후 검사를 끝내고 혈관 촬영과 pressure 측정으로 결과를관찰하여 좋은 결과를 얻었다 (Fig. 5).

Post balloon-dilatation angiography shows good patency from iliac artery to femoral artery.

Partial pressure measurment after balloon-dilatation shows increased right femoral artery pressure from 102mmHg to 154mmHg.

국소적 협착이 있고 말초부위가 이상이 없을 경우 풍선 확장으로 좋은 결과를 얻을 수 있다. 그리고 대개는 Doppler만 사용하여 사전 사후 검사도 좋지만 성공적인 혈관 성형술은 세심한 계획과 주의가 요망된다. 최근에는 좋은 기구와 함께 기본원리에 충실할 때 좋은 결과가 나타나며, 중간 크기의 환부위에 국한된 국소 병변, 특히 장골동맥은 결과가 다른 혈관에 비해 좋은 것으로 나타나 있다. 1년 개통율은 평균 90% 5년 개통율은 72%로 보고되고 있다. 시술을 전후하여 양쪽 혈압의 비교 ABI, 동맥의 파형을 보는 것이 중재적 시술을 하기전에 유용한 것으로 사려된다.
