58세 / 남자

58세/남자진행성 위암 및 췌장 전이에 대해 whipple's operation시행하고 3년 후 발생한 반복적인흑변과 혈색소 수치의 감소를 주소로 내원.

간외 문맥 협착에 의한 수입각의 정맥류 출혈

복부 컴퓨터 단층 촬영에서 간 외 문맥 협착 및 afferent loop varix와 간문맥 고혈압에 의한 이차적 소견들 즉 복수, 장벽 비후 등이 관찰되었음. 문맥 조영술에서 간 외 문맥과 상장간막 정맥에 걸쳐 심한 협착과 afferent loop에 정맥류가 형성되어 있었음(Fig.1 & Fig.2).

간 우엽의 말초 문맥을 천자한 후 8F introducer sheath를 삽입함. 이를 통해 유도철선과 5F 카테터를 전진시켜 중심 문맥에 위치시키고 문맥 조영술을 시행함. 상장간막 정맥과 비장정맥이 합류하는 부위의 중심 문맥에 90%이상의 심한 협착이 있으며 이어지는 상장간막 정맥에도 약 4~5 cm에 걸쳐 80~90%의 협착이 있음 (Fig. 2A). 3F 미세도관으로 협착 부위를 통과한 후 시행한 상장간막 정맥 조영술에서 afferent loop에 형성된 varix가 관찰됨 (Fig. 2B). 협착 부위에 대하여 10 mm x 40 mm 크기의 풍선 카테터로 혈관 성형술을 시행하였으나 (Fig. 2C), 환자가 심한 통증을 호소하며, 협착과 variceal filling이 완전히 해소되지 않았음. 고압(high pressure)으로 혈관 성형술을 지속할 경우 통증과 더불어 혈관 파열 등의 합병증이 우려되어 12 mm x 60 mm 크기의 금속 스텐트(Wallstent; Boston Scientific, Natick, MA)를 협착부위에 삽입함. 이 후 시행한 문맥조영술에서 문맥 협착이 해소되고 varix는 더 이상 조영되지 않았음 (Fig. 2D). 시술 후 간 문맥압과 상장간막 정맥압은 각각 8 mmHg와 9~10 mmHg로 유의한 차이가 없었음. 추적검사에서 환자의 흑변은 멈췄으며 혈색소 수치도 정상화 됨. Prothrombin time을 1.5-2.5 (INR) 정도로 유지하기 위해 warfarin으로 anticoagulation therapy를 2-3 개월 시행하기로 하고 퇴원함.

간 외 문맥 협착은 신생아기의 제대 패혈증, 감염, 종양, 담도계 수술, 간이식 등과 같은 다양한 원인에 의해 발생할 수 있다. 간 외 문맥이 노출되는 Whipple's operation이나 pancreaticoduod enec tomy 후 발생한 간 외 문맥 협착은 지금까지 몇 건의 증례 보고가 되어 있다. 수술 자체와 연관된 간문맥 주변 섬유화나 국소 염증이 원인으로 추정되며 혹은 choledochojeju nos tomy후 반복적인 담관염이 주변 간문맥으로 파급되어 초래할 수도 있다. 간 외 문맥의 협착은 간문맥 고혈압을 초래하여 임상적으로 소장의 variceal bleeding으로 발현할 수 있다. 이 경우 수술적 접근은 기술적으로 어려우며 대량 출혈을 유발할 수 있다. 이에 비해 간문맥 스텐트 설치술과 같은 경피적 시술은 수술에 비해 덜 침습적이며 효과적으로 문맥 혈류를 회복시키고 측부 혈관으로의 혈류를 감소시킬 수 있다. 그러나 이와 같은 간 외 문맥 협착에 대한 스텐트 설치술의 장기적 추적 결과는 아직 알려져 있지 않다.
