25세 / 남자

환자는 약 3주 전에 운동 중 right knee joint 주위에 injury를 받고 short-term splint application의 과거력이 있으신 분으로 ankle swelling 이 지속되어 doppler sonography를 시행하였고, deep vein thrombosis의 소견이 관찰되어 intervention 의뢰 되었다.

Deep vein thrombosis

Initial ascending venography에서 right calf vein과 popliteal vein, femoro-popliteal junction의 femoral vein을 involve하는 thrombosis의 소견이 관찰됨.

Right foot에 venous line을 잡고 시행한 ascending venography에서 calf and popliteal vein과 femoro-popliteal junction level까지의 femoral vein에 thrombosis의 소견이 관찰되었다. Thrombosis는 calf area에서 partial thrombosis의 소견을 보이고, 주위에서 많은 collateral vein.으로 조영제가 filling되었으며, popliteal vein은 거의 occlusion되어 있었다. 그 상방의 femoral vein 및 iliac vein에는 thrombosis의 소견 또는 다른 병발 질환의 증거는 보이지 않았다. Foot의 venous cannula를 통하여, 3일 동안 시간 당 8만 IU의 urokinase를 주입하였으며, 동시에 arm의 venous line을 통하여 시간 당 500IU의 heparin을 주입하였다 또한 다리의 su perficial vein을 collapse 시키기 위하여 foot에서부터 thigh 까지 elastic bandage를 이용하여 압박을 같이 시행하였다. 하루 뒤 시행한 추적 검사에서는 thrombosis의 소견이 여전히 남아 있었으며, 마지막 3일째 시행한 venography에서 thrombosis의 소견은 대부분이 소실된 것으로 관찰되며, collateral을 통한 flow도 감소되어 있는 것이 보여 성공적으로 치료를 끝낼 수 있었다. 환자는 이후 2일간 low molecular heparin을 치료 받았으며, coumadinization을 향후 6개월 간 치료할 계획을 받고 퇴원하였다.

하지 정맥 혈전증 환자에서 multi-side hole infusion catheter를 이용한 catheter directed thrombolysis는 intervention에서 중요한 하나의 영역으로 많은 병원에서 성공적으로 치료를 하고 있으며, 시술도 비교적 용이하기 때문에 수술적인 치료나 대증적인 치료 보다 많이 이용되고 있다. 그러나 intervention 영역에서는 popliteal fossa를 통한 popliteal vein.으로 접근을 하기 때문에 그 이하 부위에 생긴 thrombosis에 대해서는 취약하며, 특히 본 증례의 환자와 같이 thrombosis의 대부분이 calf area에 있는 경우에는 conventional sys temic heparinization으로 비교적 쉽게 치료가 되는 것으로 알려져 있어 intervention 영역에서 잘 치료를 하지 않는다. 그러나 systemic anti-coagulation은 합병증의 위험성이 높고, 완치에는 장기간의 치료를 요하는 단점이 있으며, 또한 예상외에 쉽게 thrombus가 제거 되지 않을 경우에 post-thrombotic syndrome 등이 발생하는 합병증을 일으킬 수도 있다. 이에 저자는 굳이 transcatheter method를 고집하지 않고 venography를 위해 foot에 잡은 venous cannula를 이용하여 urokinase를 직접 주입함으로써 치료 효과를 높일 수 있다는 판단 아래 시술을 시행하였으며, elastic bandage를 이용하여 calf와 thigh의 superficial vein을 collapse 시킴으로써 deep vein으로 많은 양의 urokinase가 직접 흐르도록 유도하여 transcatheter infusion thrombolysis와 비슷한 효과를 낼 수 있었다. 이런 시술을 병동에서 직접 시술하지 않고 intervention에서 시술을 한 것도 fluoroscopy guide를 이용하여 vein의 flow를 직접 봄으로써 urokinase를 주입했을 때, 그 약제가 어떤 혈관을 통하여 흐르게 될지 그래서 혈전 용해의 효과가 어느 혈관에서 잘 나타날지 예상을 할 수 있으므로 많은 도움을 얻을 수 있기 때문이다. Foot의 작은 vein을 통하여 catheter를 직접 삽입을 하고 infusion thrombolysis를 할 수도 있으나, 이는 calf의 vein이 그 크기가 작고, 많은 개수의 tributary들이 있어 잘 시도 되지 않고 있으며, 오히려 치료 성적도 기대만큼 좋지는 않을 가능성이 높다. 시술 중이나 이후에 합병증은 발생하지 않았다. 본 증례에서는 시술도 쉽고 치료 성적도 좋으나, 환자의 발등에 venous line이 있음으로 해서 환자가 이동하거나 치료를 받고 있는 동안 불편한 단점이 있었다.
