18세 / 남자

3년 전부터 운동시 우측 하지에 통증을 호소하였으며, 안정 상태에서 증상이 사라짐.

Popliteal artery entrapment syndrome

양쪽 슬와동맥 혈관조영술상 우슬와동맥의 내측 편위가 보이며, 내경 협착소견이 일부 동반되어 있다 (Fig. 1). 환자의 우측 발목을 passive dorsiflexion을 유도한 후 우슬와동맥 혈관조영술을 시행하였다. 급성기에 슬와동맥의 급격한 폐쇄소견을 보이며 (Fig. 2), 지연기에 lateral genicular artery에 의해 측부순환이 보이며 하부 슬와동맥내에 조영제가 관찰된다 (Fig. 3).

슬와동맥 포획 증후군 (Popliteral artery entrapment syndrome)은 젊은 사람에서 cramping, coldness, blanching, paresthesis, numbness, rest pain, leg ulcer 등의 증상을 보이며, 동맥 협착의 정도, 측부 순환의 발달 유뮤 등에 증상이 연관되어지며, 대부분이 만성적인 진행성 증상을 나타낸다. 원인은 선천성 기형으로 설명되며, 대부분 비복근에 의해 슬와동맥이 눌려 증상을 야기한다. 진단은 초음파을 선별검사로 이 용할 수 있으며, CT angiograhy, conventional angiography로 진단 및 확진을 할 수 있다. 초음파를 시행할 경우, 슬와동맥의 내측 편위를 정의하는 것은 어려우며, 임상소견상 이 질환이 의심되는 경우 압박 검사를 이용하면 많은 정보를 얻을 수 있다. 혈관조영술은 가장 기본이 되며 확진적인 검사법이다. 슬와동맥의 abnormal appearance, course, luminal irregularity, occlusion 소견을 주로 보인다. 또한, 시술 도중에 환자의 발을 active plantar flexion, passive dorsiflexion 하는 등으로 압박 검사를 시행함으로써 distal run-off occlusion, degree of development of collateral flow, level of reconstruction artery below knee 등을 알 수 있다. 감별진단으로는 Atherosclerosis, fibromuscular dysplasia, cystic adventitial disease, Buerger's disease 등이 있으나 특징적인 압박 검사를 동반한 혈관조영술 소견과 CT 등으로 쉽게 진단할 수 있다.
