47세/ 남자

1개월 동안 intermittent massive melena로 endoscopic sclerotherapy와 banding을 수차례 실시했으나 상기 증상이 지속됨. 내시경 소견상 식도 뿐만 아니라 위장의 cardiac portion에도 심한 정맥류가 있어서 경화요법이 어려웠음. 이학적 소견상 대량의 복수가 있었으며 초음파 검사상 간의 크기가 작아져 있고 main portal vein이 폐색되어 있었음.

간경변증과 문맥 폐색에 의한 문맥 고혈압에 동반된 위 식도 정맥류 출혈

시술 직전 시행한 초음파 검사상 portal vein thrombosis와 대량의 복수가 관찰됨. Transjugular portogram상 portal vein은 막혀 있으나 cavernous transformation에 의해 간내 문맥이 조영되었고 심한 위식도 정맥류가 보였음. Splenic vein과 left portal vein사이에 비교적 직경이 큰 collateral vein이 관찰되었음.

Ultrasonogram shows occlusion of main portal vein with small contracted liver and large amount of ascites.

Transjugular portogram shows huge variceal vein and periproctal collaterals. There is a 8 mm diameter collateral vein between splenic vein and left portal vein.

TIPS set (Cook)로 right hepatic vein에서 left portal vein을 천자하여 splenic vein과 left portal vein사이에 발달한 collateral vein을 통해 유도 철사를 splenic vein내로 삽입한 후 5F 카테터로 portogram을 실시함. portogram상 약 8mm 직경의 collateral vein을 확인하고 10mm 직경의 풍선 카테터 (Meditech)로 tract을 확장한 후 직경 10mm, 길이 69mm, Wallstent (Schneider)와 직경 10mm, 길이 50mm, Hanaro stent (Sooho meditech)를 collateral vein과 left hepatic vein 사이에 삽입하였음. Pre- and post-TIPS portogram상 variceal flow가 현저히 감소하고 hepatic perfusion은 완전히 소실되었음.

Post-TIPS portogram shows reduced variceal vein size and flow. Two stents, a Wallstent, 10mm diameter, 69mm length, and a Hanaro stent, 10 mm diameter, 50 mm length, were introduced between collateral vein and right hepatic vein.

Portal vein thrombosis가 있는 환자의 TIPS는 기술적으로 어려운 것으로 알려져 있으나 대부분 collateral circulation에 의해 intrahepatic portal vein은 patency가 유지되어 있어 천자하는데 어려움이 없고 thrombosed portal vein 내로나 collateral circulation을 통하여 유도철사를 splenic vein까지 삽입할 수가 있다. 저자들의 경우 최근 5예의 portal vein thrombosis에 의한 portal vein occlusion환자 전예에서 TIPS를 실시할 수가 있었으며 4예에서는 occluded portal vein내로 스텐트를 삽입하였고 본예에서는 collateral vein내로 스텐트를 삽입할 수가 있었다. Portal vein occlusion이 없었던 환자에 비하여 특별한 시술상의 어려움은 없었으나 경로가 길기 때문에 한개 이상의 스텐트가 필요해서 경제적 부담이 문제로 생각된다. 내시경적 치료로 지혈되지 않으며 portal vein thrombosis가 있는 정맥류 출혈 환자에게도 TIPS는 좋은 치료 방법으로 이용될 수 있을 것으로 생각된다.
