56세/남자

약 2개월 전에 간 이식 수술 후 발생한 Jackson-Pratt (JP) drain을 통한 출혈로 의뢰 받음.

Hepatic artery pseudoaneurysm after living donor liver transplantation

조영증강 복부CT에서 JP drain에 인접한 간동맥에 1.5 cm 크기의 가성동맥류가 있었다 (Fig 1A, 1B).

5 Fr. Yashiro catheter로 복강동맥조영술을 시행하고 (Fig. 2A), 8 Fr. RDC guiding catheter로 교체한 후 0.014-inch guide wire를 따라 4 mm-1.9 cm balloon-expandable stent-graft (Jostent GraftMaster, Abbott Vascular Instruments, USA)를 간동맥 가성 동맥류를 포함시켜 설치하였다 (Fig 2B). 시술 직후 시행한 복강동맥조영술 (Fig. 2C)과 추적 조영증강 복부CT에서 가성동맥류는 더 이상 보이지 않았고, 간동맥 혈류에 이상 소견도 없었다(Fig 3A, 3B).

간동맥의 가성동맥류는 간이식 후 약 1~2% 에서 발생하는 매우 드문 합병증이지만, 사망률이 매우 높은 치명적인 합병증이다. 가성동맥류는 대부분 간이식 후 한 달 이내에 발생하며 복부 통증이나 다량의 상부위장관 출혈 및 쇽 등의 증상을 동반한다. 가성동맥류가 발생하였을 때 고전적인 치료법으로 외과적 절제와 혈관 문합, 또는 경피적 코일색전술이 있다. 그러나 외과적 절제와 혈관 문합을 시행하는 경우 간 문부 혈관의 유착과 염증이 발생 할 수 있고, 코일색전술의 경우 시술 자체가 실패하거나 시술 후 간으로 가는 혈류량이 감소하여 이식된 간의 허혈을 초래하는 경우가 있다. 최근 2000년도 이후 새로운 치료법으로 stent graft를 시술한 증례가 보고가 발표되고 있으며, 아직 장기 추적에 대한 보고는 없으나 간이식 후 생기는 간동맥의 가성동맥류 치료에 보다 효과적일 것으로 기대한다.
