Stent graft, Fistula, Pseudoaneurysm

34세/남자

4-5번째 요추 추간판탈출 수술 후 발생한 복부의 잡음을 주소로 내원함.

Common iliac arteriovenous fistula (AVF) with pseudoaneurysm formation

CT volume rendering 영상에서 우측 총장골 동맥에 가성동맥류가 있고 동측 총장골 정맥과 하대정맥이 동시에 조영되어 동정맥 누공이 확인되었다 (Fig 1A).

좌측 총대퇴동맥을 통해 5Fr. Omniflush catheter를 삽입하여 골반혈관촬영을 시행한 후(Fig. 1B), 0.035-inch guide wire를 이용하여 우측 총장골 동맥의 가성동맥류를 지나 우측 총장골동맥의 원위부로 삽입을 시도하였으나 용이하지 않았다. Guide wire의 말단부를 가성동맥류 내에 위치시킨 후 우측 총대퇴동맥을 통해 10Fr. Vascular sheath를 삽입하였다. 이를 통해 snare technique으로 guidewire를 잡은 후, through and through technique으로 12 mm-6 cm self-expandable stent graft (Niti-S, Taewoong, Korea)를 가성동맥류를 포함한 우측 총장골동맥에 설치하였다(Fig. 2A, B). 이후 시행한 골반혈관촬영에서 동정맥루와 가성동맥류는 더 이상 조영되지 않았다(Fig. 3).

주요 혈관 손상은 요추 추간판 수술의 드문 합병증으로, 1945년에 척추궁 절제술 이후 발생한 우측 장골동맥과 대정맥 사이의 동정맥루가 처음으로 보고되었다. 총장골정맥은 네번째 요추의 우측 전측부에서 합류되어 대정맥를 형성한다. 또한 대동맥은 척추에 대해서 약간 좌측으로 위치하고 있으면서 네번째 요추의 상방에서 분기하고 있다. 이러한 해부학적 구조로 인하여 추간판 수술의 합병증으로 동정맥루가 발생하며 이때 가성동맥류를 동반하는 경우도 있다. 과거 동정맥루에 대해 외과적 수술을 시행하였으나 1990년대 이후 새로운 치료법으로 stent graft 삽입술이 성공적으로 시행되어 왔고, 외과적 수술에 비해서 덜 침습적이면서 시술 이후 사망률과 이환율 역시 적다.
