50세/남자

과거에 심한 음주력을 가진 상기 환자가 3~4일전부터 시작된 흑혈변과 어지럼증을 주소로 내원하여 시행한 복부 전산화 단층 촬영술상에서 궤양성 위출혈을 진단받고 혈관조영술을 시행받았다.

복부 전산화 단층 촬영술상에서 위각부 소만곡 부위에서 조영제의 누출이 있고, 조영제가 dependent portion을 따라 흐르고 있는 양상을 보이고 있다. 복강동맥 조영술 상에서 위가 혈종으로 인해 팽만된 양상을 보이고 있다. 위각부에서 시작되어 위체부 소만곡의 윤곽을 따라 조영제가 위 내강 내로 유출되는 소견을 볼 수 있는데, 그 양상이 혈관으로 오인을 할 수 있는 소견이다. 비위관을 통해 위세척을 한 후 시행한 복강동맥 조영술 상에서 불규칙하게 퍼져나가는 양상의 위궤양출혈을 쉽게 인지할 수 있다. 두 군데의 bleeding focus가 있었고, gelfoam과 glue를 이용하여 색전술을 시행하였다.

RH catheter를 이용하여 복강동맥조영술을 시행하였다. 이 후, microferret catheter를 이용하여 좌위동맥 조영술을 시행하였다. 좌위동맥 조영술상에서 두 군데의 bleeding focus가 있었고, gelfoam과 glue를 이용하여 색전술을 시행하였다.

궤양성 위출혈로 인해 많은 양의 혈종이 위 내강 전체를 채우게 될 때, 출혈은 혈종과 위 벽의 사이 공간을 따라 흐르게 되며, 혈관조영술 상에서 마치 정맥 내부로 조영제가 흐르는 것처럼 보일 수 있다. 위 세척을 하면, 혈종이 다소 줄어들어 퍼져나가는 출혈을 확인 할 수 있다.
