2005년 peripheral artery stenosis 진단후 Lt. balloon angiography, Rt. subintimal dissection with stent insertion 시술 받은 환자로 외래로 경과 관찰중에 양측 발의 피부색 변화와 coldness, calf pain호소.

08년 2월 20일 날 시행한 Lower abdominal aortography 에서 total occlusion 이 좌측 external iliac artery에서 관찰되었고 left side에서 Ext. iliac a. 부터 common femoral a., supf. femoral a. 에 걸쳐 occlusion이 관찰되었다. 6mm - 4cm sized balloon 을 이용하여 superfical femoral artery 부터 common iliac artery 까지 balloon dilatation 시행하고 8mm - 8cm sized stent 를 Rt. superficial femoral artery에 사용, 10mm- 8cm sized stent 를 Lt. common iliac artery부터 Lt. common femoral artery에 설치 하였다. stent 설치 후에 angiography 에서 distal common femoral artery에 혈류가 관찰되지 않고 remained stenosis 가 common femoral artery에서 보여 6mmx4cm size의 풍선을 사용하여 여러 번 확장을 시행하였으나 혈류의 흐름에는 호전이 없어 10mm x 6cm size 의 stent를 사용하여 dilatation시킨 후 시행한 혈관조영상에서 좌측 common iliac artery 부터 좌측 posterior tibial artery를 거쳐 plantar artery 까지 혈류가 잘 관찰됨.

Artery의 stenosis에서 revascularization의 방법중 비수술적 intervention방법으로 그동안 PAT, stent 삽입술, 죽상반 절재술이 이용되어 왔고 특히 iliac a. PTA의 초기 개통률은 95%이며 stent를 삽입할 경우 개통률은 더욱 높아진다. 3년 개통률 은 70%를 조금 넘는다. 말초 혈관의 질환에 대한 경피적 중재적 시술법은 mortality를 최소화 하고 80%의 환자에서 2년간의 개통률을 보이며 limb-threat, limb-salvage 비율을 낮추어준다. 특히 Percutaneous infra-inguinal revascularization 방법은 chronic lower extremity ischemia가 있는 환자에서 일차적 치료법으로 고려되어야 한다. 또한 65세 이상의 노인 환자에서 PTA/S방법은 GSVB방법과 비교하여 비슷한 5년 개방성이 비슷하고 mortality가 낮으므로 충분히 일차적 치료 방법으로 고려해 볼만하다. 다만 medical treatment와 비교하여 초기 6개월의 성적은 유의하게 좋지만 장기간(>1year )에 대해서는 삶의 질이나 장시간 보행에 대한 조사에서는 유의한 차이를 보여주지 못한다는 결과가 있어 꾸준한 추적 관찰이 필요하다.
