Aneurysm, arteries, interventional procedures, therapeutic embolization

67세/남자

3주전 Carpal tunnel syndrome으로 endoscopic carpal tunnel release 수술 한 후, painful swelling 발생하여 내원함. 이학적 검사에서 오른손의 hypothenar area에 localized tenderness, 박동성 종괴, 경도의 ecchymosis가 있음.

자동맥 가성동맥류

오른쪽 손바닥에서 시행한 Color Doppler 초음파에서 우측 자동맥의 palmar segment에서 origin하는 가성동맥류(21mmx31mmx15mm)가 있음 (Fig. 1).

초음파 유도하에 자동맥의 가성 동맥류를 21 게이지 나비 바늘로 천자한 다음 혈관조영술을 시행하였으며 혈관조영술에서 자동맥의 palmar segment에 발생한 small neck를 보이는 가성동맥류가 있음(Fig. 2). 경피적 thrombin injection을 계획하였으며 distal embolism을 예방하기 위하여 proximal ulnar artery를 continuous manual compression을 한 다음 1 ml tuberculin syringe에 thrombin을 받아 500 unit를 injection하였음. 이 후 시행한 color Doppler 초음파 검사에서 가성 동맥류는 thrombus formation을 보이며 obliteration되었음(Fig. 3).

손에 발생하는 가성동맥류는 드물며 penetrating injury나 iatrogenic manipulation에 의해 발생한다. 이에 대한 classical treatment는 수술이지만 non-surgical treatment로 초음파 유도 하에 compression하는 방법, 초음파 유도 하에 색전술 등이 소개되어 있다. 초음파를 이용한 compression은 60-90%의 cases에서 효과적이라고 보고하였으나 time consuming하며 painful하고, 크기가 큰 가성동맥류에서는 적합하지 않은 단점이 있다. 초음파 유도 하 thrombin injection은 femoral artery에 발생한 pseudoaneurysm에서 93-100%의 성공률을 보고하고 있으며 최근 보고에 의하면 다른 부위의 가성 동맥류 치료에 사용되고 있다. Small vessel의 pseudoaneurysm은 systemic thrombin administration, intravascular thrombosis, distal ischemia등의 위험이 있다. 이러한 risk를 줄이기 위해 천자하는 바늘 끝은 가성 동맥류의 neck에서 최대한 멀리 위치하게 해야 하며, 지속적인 color Doppler control하에 소량의 thrombin을 천천히 injection하여야 한다.
