Diabetic vasculopathy; lower extremity artery, angioplasty

68세/남자

20년간 당뇨병으로 치료받아오다가 1개월 전부터 왼쪽 발가락에서부터 non-healing wound 와 gangrene이 생기고 발등까지 점점 infection이 파급되고 있는 상태에서 내원함

Diabetic vasculopathy of lower extremity arteries

Left common femoral artery(CFA)를 antegrade access하여 6F vascular sheath(Seongwon, Cheongju, Korea)를 삽입하고 angiography를 시행함. CFA와 superficial femoral rtery(SFA)에 여러군데 다발성 협착이 있고 popliteal artery(PoA)의 근위부는 chronic total occlusion(CTO)되어 있으며 distal popliteal artery(DPA)는 희미하게 조영된다. Leg에서는 posterior tibial artery(PTA)가 비교적 보존되어 있어나 ankle level에서는 군데군데 occlusion되었다가 lateral plantar artery(LPA)로 연결되고, anterior tibial artery(ATA)는 원위부의 일부만 조영되며 dorsalis pedis artery는 전혀 보이지 않는다.

CFA의 협착부는 6mm cutting balloon(Boston scientific, Natick, USA)로 angioplasty하였다. PoA의 segmental CTO는 3mm balloon(Savy, Cordis, Miami, USA)와 0.016인치 microwire (GT; Terumo, Tokyo, Japan)조합을 이용하여 subintimal passing한 다음 angioplasty(3mm, 5mm)하고 residual stenosis가 의미있게 남아 6mm self expandable stent(Smart; Cordis)를 설치하였다. PTA는 원위부의 다발성 국소 폐쇄부분을 역시 balloon(Savy, Cordis)와 0.016인치 microwire(GT, Terumo)조합을 이용하여 subintimal passing하여 LPA까지 진입한 다음 2mm/3mm angioplasty 하였다. ATA는 기시부와 DPA가 전혀 보이지 않아 subintimal passing을 할 수 없었다. Completion angiography상 CFA, SFA, PoA, PTA를 통한 혈류 및 left foot으로 혈류가 현저히 개선됨을 확인 할 수 있었다. 이후 gangrene이 생긴 제2족지 는 제거하고 발 병변의 나머지 부분은 보존적 치료로 회복되었다.

당뇨족의 경우 neuropathy가 있거나, vasculopathy가 있거나, 아니면 복합적인 경우로 나누어서 치료적 접근을 하여야 한다. Vasculopathy의 경우 기본적인 병리적 변화는 동맥경화이다. 다만 그 정도가 심하고, 분포가 광범위한 경우가 많다. 특히 PoA이하의 crural arteries들이 심하게 침범되는 경우가 많으므로 이 부분의 혈류를 개선시키기 위한 치료가 필요하다. 일단 발에 상처나, 궤양, 괴사 등의 병변이 생기면 상처회복을 위해서는 평소의 1.5배 이상의 혈류가 필요하다. 연구에 의하면 곁혈류 등에 의하여 ATA나 PTA의 원위부가 보여도 이 정도로는 치료에 불충분 하므로 발까지 가는 직선혈류(straight flow)를 회복시켜 주는 것이 큰 도움이 된다고 한다. 위 증례처럼 진행된 당뇨족일지라도 적극적으로 혈관치료를 시행한다면 발의 병변이 회복되거나, 절단해야 할 부위를 최소화 시킬 수 있다.
