Rectal varix, embolization

알코올성 간경화를 앓고 있는 환자가 내원 2일전부터 rectal bleeding을 주소로 내원함. Sigmoidscopy 상 rectal varix 보이나 지혈하는데 실패함. 내원 당시 Hb 5.8 g/dL, platelet 90,000/uL, total bilirubin 18.4 mg/dL, INR 2.94 보임, TIPS를 하기에는 liver dysfunction 이 심하고 esophagus 나 fundus 에는 varix가 보이지 않음.

Isolated rectal varices

CT scan에서 rectum에서 varix에 의한 active bleeding 소견이 있음(Fig. 1).

투시하에 간문맥 말초부위를 경피적으로 천자하여 5Fr sheath를 삽입하였다. 5Fr Davis catheter를 SMV에 위치시키고 시행한 portogram 상, IMV가 확장 되어 있고 rectum으로 hepatofugal blood flow를 보이며 rectum에서 varices를 형성한 후 (Fig. 2~3) internal iliac vein 으로 drain 되고 있음. Microcatheter로 superior hemorrhoidal vein을 selection 한 뒤, 양측 vein을 Gelfoam으로 embolization 한 후 flow가 느려졌을 때 2:1(0.5cc glue, 1 cc Lipiodol) glue-lipiodol mixture를 이용하여 embolization 함 (Fig. 4). Completion angiography 에서 IMV로의 flow는 차단되어 보이지 않음. Puncture site 는 2:1 glue로 embolization한 뒤 시술을 종료하였다. 시술 후 직장 출혈은 즉시 멈추었고 3일 뒤 환자는 퇴원 할 수 있었다.

Esophagogastric 이외의 ectopic varices 는 간문맥 항진증 환자에서도 드물다. Ectopic varices 중 rectal varices는 흔하면서도 이로 인한 정맥의 파열은 보고가 드물지만 치명적일 수 있다. 어떤 보고에서는 간문맥 항진증 환자를 대상으로 직장 내시경을 하였을 때 44 -90% 환자에서 anorectal varices를 관찰할 수 있었고 이 중 10%에서 출혈을 일으키는 것으로 보고하였다. Rectal varices의 standard treatment는 아직 정립되어 있지 않으나 endoscopic variceal ligation(EVL), endoscopic injection sclerotherapy(EIS), transjugular intrahepatic portosystemic shunt(TIPS), transhepatic or TIPS approach 후 embolization, 그리고 수술적인 방법이 있을 수 있겠다. 이 중 EIS 경우, large varices의 경우 희석 효과로 인해 그 결과가 만족스럽지 않을 수 있다. Ibukuro 등은 확장된 paraumbolical vein을 통하여 rectal varices를 치료하였다고 보고하였다. 색전물질로는 coils, gelatin sponges, sterilized ethanol, glucose, and ethanolamine oleate, glue 등을 다양하게 이용할 수 있겠다. 본 증례의 경우 TIPS를 시행하기엔 시술 후 간부전 가능성이 높아 고려하지 않았다. 경피적 간문맥 천자는 이런 환자의 경우 출혈 가능성이 높지만 천자 부위를 glue를 이용하여 성공적인 지혈을 얻을 수 있었다.
