Inferior vena cava filter, inferior vena cava thrombosis

60세/남자

환자는 4개월 전 좌측 하지 부종을 주소로 내원하여 시행한 CT angiography에서 좌측 총장골정맥(common iliac vein)에서 하대정맥(inferior vena cava, IVC)으로 이행부에 IVC 내강으로 돌출한 부유혈전(floating thrombi)과 좌측 대퇴정맥(femoral vein)의 thrombi가 진단되었고 폐색전의 예방을 위하여 IVC filter (Gunther Tulip ; COOK, USA) 삽입술을 시행받고 항응고요법을 시행하였다. 환자의 증상은 호전되었으나 3주 후 시행한 추적 CT에서 IVC thrombi가 남아있어 filter를 제거하지 않고 퇴원하였으며 wafarin을 투여 받으면서 외래에서 추적 관찰 중 2주 전부터 환자 임의로 wafarin 복용을 중단하였다. 내원 1일 전부터 양측 엉덩이 통증이 발생하여 응급실로 내원하였다.

하지 CT angiography에서 IVC filter가 삽입된 부위를 포함하여 그 이하로 IVC와 양측 common iliac vein 내강을 확장시키는 thrombi가 있다(Fig. 1). 이전의 좌측 femoral vein의 혈전은 더 이상 보이지 않았다.

양측 femoral vein을 천자 후 venogram에서 양측 common iliac vein이 thrombi에 의해서 막혀있고 측부순환이 일부 발달된 상태였다(Fig. 2). Heparin 3000 unit를 정맥주사 후 5F angiographic catheter를 사용하여 thrombi 내부에서 UK 50만 unit를 주입하였으며 이후 양측 iliac vein으로부터 IVC에 걸쳐 pulse spray catheter (Multi-Sideport Catheter; COOK, USA)를 위치시키고 좌우 각각 시간당 5만 unit의 UK를 infusion하기로 하고 병실로 전원하였다. 약 24시간의 overnight thrombolysis 후 시행한 follow-up venogram에서 thrombi의 대부분은 보이지 않고 IVC filter 주변, 좌측 common iliac vein과 IVC에 걸쳐 일부 thrombi가 남아있는 소견이 보였다. Long sheath를 사용하여 남아있는 thrombi에 대하여 aspiration thrombectomy를 시행하였고 이후 얻어진 venogram에서 thrombi의 대부분이 제거되었고 혈류장애 보이지 않아 시술을 종료하였다(Fig. 3). 환자는 경구 wafarin을 유지한 상태로 시술 후 2년이 지난 현재까지 특별한 문제 없이 외래 추적관찰 중이다.

IVC filter는 심부정맥혈전증 환자에서 폐색전을 예방하기 위해 고안된 기구로, 1967년 Mobin-Uddin umbrella filter가 처음으로 개발된 후 다양한 형태의 filter들이 개발되고 있다. 최근에는 삽입 후 폐색전의 위험성이 없어졌을 때 제거할 수 있는 retrieval filter가 개발되어 광범위하게 사용되고 있다. Retrieval filter를 사용하였을 때 제거할 수 있는 시기는 삽입 후 약 2주 이내이다. 삽입기간 동안 발생할 수 있는 합병증은 폐색전, 하대정맥혈전증, filter의 이동, 정맥부전, filter의 골절 등이다. 이 중 하대정맥 혈전증의 발생률은 약 0-28% 정도의 넓은 범위로 보고되고 있다. 그 원인은 filter 자체가 foreign body로 혈전의 형성을 조장하기 때문인 것으로 추정된다. IVC filter와 연관된 혈전증의 치료는 다양한 선택이 가능하며, systemic anticoagulation, systemic thromboly tic thera py, surgical thrombectomy, catheter-directed thrombolysis 및 endovascular mechanical thrombectomy 등이 시행될 수 있다. Systemic anticoagulation 4 sy stemic thrombolytic therapy는 iliocaval thrombosis에 대한 효과가 적다. Surgical thrombectomy는 효과적이긴 하나 매우 침습적인 시술이어서 제한적으로 시행되고 있다. 반면 인터벤션 치료인 catheter-directed thrombolysis와 endovascular mechanical thrombectomy의 경우 덜 침습적이면서 효과가 뛰어나 현재 널리 시행되고 있는 치료 방법이다. 본 증례 역시 IVC filter 삽입 후 재발한 IVC thrombosis에 대해 인터벤션 시술이 이루어졌으며 성공적인 치료가 가능하였다.
