임상소견
8개월전 urinary bladder cancer (transitional cell carcinoma) 진단받고 transurethral resection of tumor, chemotherapy 및 radiation therapy받은 환자로 gross hematuria 재발되어 내원
검사소견
Hgb 3.8 gm/d, Hct 12%
진단명
Recurrent bleeding of urinary bladder cancer
Right internal iliac arteriogram상 right inferior vesical artery에서의 종양 출혈을 확인(Fig.1).

시술방법 및 재료
5F sidewinder type catheter를 이용하여 right internal iliac angiography시행후 3F microcatheter (Microferret, Cook)을 이용하여 right inferior vesical artery의 출혈부위를 selection한 후 coils, Gelfoam particles및 mixture of ethanol and Lipiodol를 이용하여 embolization시행 (Fig.2)한 후 left inferior vesical artery도 Gelfoam으로 embolization시행함. 이후 출혈의 징후 없이(Fig.3) 외래 follow-up 중.

고찰
전반적 상태가 좋지 않은 Tumor bleeding 환자에서 경동맥 색전술은 신속한 지혈효과와 환자의 빠른 회복을 유도함으로써 morbidity와 mortality가 높은 수술적 방법을 대체하고 있다. Internal iliac artery는 풍부한 collateral blood supply를 받기 때문에 광범위한 embolization에도 불구하고 안전하게 지혈을 시행할 수 있는 반면 collateral blood supply에 의한 재출혈의 가능성도 그만큼 많은 것으로 되어 있다. Gelfoam 만 사용한 경우 2-3주 후에 recanalization되므로 coil과 ethanol을 함께 사용하여 충분한 양의 embolization이 필요하다.

본 증례에서는 출혈 혈관인 right inferior vesical artery를 선택하여 다양한 색전물질로 색전하였습니다. 이중 무수알콜도 사용되었는데 색전혈관의 영구적 폐쇄에 도움이 될 수 있습니다. 그러나 internal iliac artery에서 출혈에 대한 색전을 시도할 경우에는 무수알콜을 사용하면 주위 장기와 조직에 광범위한 손상을 초래하므로 사용되지 않고 Gelfoam particle만이 주로 사용됩니다.
