chemoport tube, loop-snare catheter, and pigtail catheter

48세/남자

Rectosigmoid cancer with liver metastasis로 수술 후 chemoport insertion한 환자로(Fig. 1) 6 개월 후 복부 CT에서 chemoport tube의 절단이 관찰되었음(Fig. 2). 2개월 후 경피적 제거술을 시행함.

단순 흉부촬영상 관찰되는 잘려진 chemoport tube가 left pulmonary artery에서 longitudinal하게 위치하며 보이며 양 끝부위가 left pulmonary artery의 branch 에 닿아있음 (Fig. 2).

18G angiocath needle을 사용하여 초음파 유도하에 우측대퇴정맥을 후향 천자하였으며 10Fr Terumo vascular sheath를 삽입한 후 5Fr headhunter catheter를 이용하여 left pulmonary arteriogram을 얻었음(Fig. 3) . 그리고 0.035”Terumo guide wire와 5Fr Cordis pigtail catheter를 이용하여 longiduinal하게 위치한 잘려진 tube를 pulmonary artery의 wall로 부터 혈관 안쪽으로 당겨내고 pigtail catheter를 twist 시켜 tube와 교차시켰음(Fig. 4A and B). 그리고 pigtail catheter를 따라서 Loop-Snare를 벌려 loop 사이로 pigtail catheter를 넣어 폐동맥까지 이동시켜 잘려진 tube까지 도달하게 하였음. 그 후 Loop-snare를 이용하여 pigtail catheter와 잘려진 tube를 한꺼번에 포획하여 체외로 제거하였음 (Fig. 4C and D).

Central line or venous port의 삽입은 영상의학과 인터벤션 보다 외과적 삽입의 비율이 더 큰 실정이다. 그러나 blind subclavian venous access 자체는 Pinch-off syndrome 의 위험성을 동반한다. 본 증례의 경우 pinch-off syndrome이 발생하고 있는 subclavian venous central line을 통하여 chemoport의 tube가 삽입된 지 약 6개월 이내에 tube의 절단이 일어났으며 최소 40일 이상 환자는 무증상이었다. 폐동맥내의 이물질은 Thanigaraj등이 보고한 바와 같이 10년 이상 잘려진 catheter가 무증상으로 존재할 수 있으나 Espiritu 등이 보고한 바와 같이 pulmonary hypertension과 pulmonary arterial thrombus with severe tricuspid regurgitation을 유발할 수도 있다. central venous catheterization의 access route는 Pinch off syndrome을 유발할 수 있는 subclavian vein을 사용하기보다는 jugular vein을 사용하는 것이 합병증을 줄일 수 있을 것으로 생각된다.
