Epigastric and RUQ pain으로 내원하여 초음파상 mass 발견

S8 segment에 위치한 간암으로 간의 변연부에 위치하고 있으며(Fig.1), 이전의 간동맥 화학색전술로 인한 Lipiodol이 종괴의 내측면에는 잘 섭취되어 있으나, 외측면에는 Lipiodol 섭취가 보이지 않아 (Fig.3) 간암의 측부혈행에 의한 영양혈관이 있을 가능성을 의심하여 동맥화학색전술의 재시도를 계획하였다.

There is a wedge-shaped enhancing mass which is extended over hepatic capsule with indistinct tumor margin.

The celiac angiography shows hypervascular tumor vessels with dense tumor-staining on the S8 of liver.

The CT shows incomplete uptake of Lipiodol along the outer peripheral margin of the tumor. There is a linear feeding vessel along anterior abdominal wall. It is suggestive of a feeding vessel from anterior intercostal branches of rt. internal mammary artery (arrow).

간암의 혈액공급의 가장 흔한 혈관인 간동맥을 검사하여 종양의 영양혈관이 없음을 확인 후 측부혈행의 가장 흔한 inferior phrenic a., omental artery를 검사하였다. 이러한 혈관에서 종양의 영양혈관이 없다는 것을 확인 후 posterior intercostal a.를 선택하여 영양혈관을 검사하였다. 그러나 이러한 혈관에서도 영양공급 혈관을 발견할 수 없었다. 1차 TAE후 32주 control spiral CT의 arterial phase를 주의 깊게 관찰하여 anterior intercostal a.의 비후가 의심되어 right internal mammary a.를 선택 조영하였다. Internal mammary a.의 DSA혈관 조영상 distal branches인 anterior intercostal a.의 혈관에서 종양의 염색을 확인할 수 있었다. S-P microcatheter를 이용하여 종양 영양공급혈관을 초선택하여 Adriamycin 50mg, Lipiodol 7cc를 emulsion으로 만들어 주입한 후 Gelfoam particle을 1x1x2mm 정도의 크기를 만들어 추가색전을 실시하였다.

There is prominent Lipiodol washout in anterosuperior portion of mass lesion by the regrowing of tumor.

It shows hypervascular tumor staining supplied by anterior intercostal branches of right internal mammary artery.

There is a mass with well-uptaken Lipiodol on the RUQ area.

TAE는 HCC의 치료시 수술 이외에 사용하는 방법으로 널리 알려져 있다. 대개 간동맥에서 혈류공급을 받게 되나, 수술적 결찰이나 색전술, 또는 다른 여러가지 원인에 의해 간외측부혈행이 일어나게 되고, pancreaticoduodenal arcade, periportal roue, intercostal artery, left gastric artery, inferior phrenic artery, paracolic gutter route and omental branches들이 주경로들이다. 간동맥의 충분한 devascularizatio에도 불구하고, 지속적인 AFP의 증가가 있거나 Lipiodol을 infusion하고 난뒤, CT나 plain film에서 viable tumor 내에 Lipiodol의 focal defect가 존재시 다른 측부혈행의 존재를 시사한다. 간동맥의 anterosuperior aspect와 diaphragm이나 anterior abdominal wall에 인접한 부위에 tumor가 존재시 internal mammary artery를 feeding vessel로 의심해야 될 것으로 사료된다.
