30세/ 남자

간암으로 2-3개월 간격으로 3번의 동맥색전술을 받고 퇴원 후 fever를 주소로 응급실을 찾아옴

입원 당시 시행한 CT 소견상 우하간엽에 Lipiodol 침착이 있고 그 후방에 water density의 cystic lesion이 관찰됨 (Fig.1). 다음날 시행한 초음파 검사상 간우엽에 cystic fluid collection이 관찰되었고 aspiration에서 biloma를 확인함(Fig.2).

Biloma가 fever의 원인으로 생각되어 배액술을 실시하기로 하였다. 초음파검사로 천자 부위를 정한 뒤 투시 유도하에 Seldinger technique으로 cyst내에 8F pigtail catheter를 삽입하였고 (Fig.3) 시술 다음날 환자의 체온은 정상으로 돌아왔다. 두달 후 투시유도하에 pigtail catheter를 제거하였다.

Percutaneous Drainage was done with 8F pigtail catheter in biloma of liver.

이 환자의 경우 동맥색전술시 chemohterapeutic agent (Adriamycin 40mg, Mitomycin 10mg), Lipiodol emulsion과 Gelfoam powder를 사용하였다. Gelfoam powder는 본원에서 Gelfoam mixer기계로 갈아 만들어서 소독후 일정양을 사용하였다. 동맥색전술시 embolic material로 Gelfoam powder를 사용할 경우 종양 괴사 효과는 더욱 뚜렷하나 250 micrometer보다 작은 particle이 peribiliary plexus, sinusoid등을 막아 biliary tree, normal liver parenchyme 등에 ischemic necrosis를 때로 초래하는 것으로 알려져 있고 bile duct necrosis때는 손상된 bile duct 주위로 biloma를 형성한다. 그 외에 Gelfoam powder는 Gelfoam particle보다 abscess, gall bladder necrosis를 잘 일으키는 것으로 알려져 있다. 따라서 간세포암의 동맥색전술시 embolic material의 크기는 최소한 250 micrometer 이상은 되어야 한다.
