Living donor liver transplantation, gastric varix, bleeding, N-butyl cyanoacrylate

생체간이식 수술 후 간기능검사는 정상이나 추적 내시경에서 위정맥류의 호전이 보이지 않았음. 수술 6개월 후 갑자기 대량의 토혈(2000cc가량)을 주소로 응급실을 내원하였으며 당시 수축기 혈압은 60mmHg이었고 심박수는 125회였음. 응급내시경으로 위정맥류출혈을 확인하였으나 다량의 출혈로 인해 지혈에 실패함.

생체간이식 수술 4개월 후 시행한 복부 CT에서 위정맥류와 위신단락(gastrorenal shunt) 소견이 관찰됨(Fig. 1A, IB). 간내, 간외 문맥은 정상이며 간정맥도 개통되어 있음. 대량의 토혈로 응급실 내원 당시 시행한 내시경검사상 활동성 출혈 (active bleeding)이 관찰됨 (Fig. 2).

초음파 유도 하에 우측 문맥 분지를 천자하여 시행한 문맥조영술상 후위정맥(posterior gastric vein)과 좌위정맥(left gastric vein)을 통해 위정맥류가 관찰됨(Fig. 3). 선택적 후위정맥조영술상 위정맥류 파열에 의해 위장으로 조영제 일출(contrast extravasation) 소견이 관찰됨 (Fig, 4). N-butyl cyanoacrylate (histoarcyl, B. Braun, Melsungen, Germany)와 lipiodol(Guerbet, Aulnay-sous-Bois, France)을 1 : 5 비율로 혼합하여 위정맥류와 위신단락까지 색전하였으며 후위정맥과 좌위정맥도 histoacryl로 색전하였음. 색전술 후 시행한 문맥조영술상 위정맥류는 관찰되지 않으며 hepatofugal flow도 소실되었고 간내 문맥 혈류도 원활함 (Fig. 5). 5 Fr 카테타를 간외 문맥에 위치시킨 후 측정한 문맥압은 11 mmHg로 임상적으로 의미있는 문맥고혈압은 없었음. 경피경간 천자 경로는 1：2 비율의 histoacryl과 lipiodol 혼합액으로 성공적으로 색전하였음. 시술 후 출혈은 바로 멈추었으며 추적 내시경에서도 위정맥류출혈의 소견은 관찰되지 않았음.

위정맥류출혈의 중재적 치료법에는 perctuanous transhepatic varix obliteration (PTVO), transjugular intrahepatic portosystemic shunt (TIPS), balloon occluded retrograde transvenous obliteration(BRTO)등이 알려져 있다. PTVO는 문맥고혈압을 감압시키지 못하기 때문에 재발이 잦아 TIPS의 보편화 이후 시술 빈도가 줄어들고 있으며 위정맥류의 치료에는 주로 TIPS와 BRTO가 많이 시행되고 있다. 그러나 생체간이식을 받은 환자의 경우 TIPS시술이 기술적으로 어려우며 시술 후 간 괴사, 간정맥혈전증, 간성 뇌병증의 발생 가능성이 있어 주의하여야 한다. BRTO의 경우 위신단락을 가진 생체간이식환자의 위정맥류 출혈의 치료에 있어 TIPS에 비해 안전하게 시행할 수 있으며 위신단락을 막아줌으로써 간내 문맥 혈류를 증가시킬 수 있는 장점이 있다. 그러나 대량의 위정맥류 출혈시 위신단락을 풍선카테타로 막을 경우 위정맥류 파열을 조장할 뿐만 아니라, 출혈된 위정맥류의 경화효과를 얻기 위해서는 시간이 필요하다는 문제점이 있어 대량의 활동성 위정맥류출혈에 있어서는 시술에 어려움이 있다. 최근 histoacryl을 이용한 내시경 혹은 경피적 색전술을 통해 위정맥류출혈에 대한 즉각적인 지혈효과를 얻었다는 보고들이 발표되고 있으며 재발율 등 장기 효과에 대한 보다 많은 연구가 필요하겠지만 정맥류 자체를 histoacryl로 완전히 채운다면 재발률을 낮출 수 있을 것으로 생각된다. 또한 본 증례와 같이 생체간이식후 문맥고혈압이 교정된 상태에서는 경피적 histoacryl 색전술만으로도 효과적인 장기 결과를 이룰 수 있을 것으로 생각된다. 결론적으로 본 증례는 TIPS악 BRTO 시술이 용이하지 않은 생체간이식환자에서 내시경으로 조절되지 않는 급성 대량 위정맥류출혈에 있어 histoacryl을 이용한 경피적 위정맥류 색전술은 대안 치료로 유용하다는 것을 보여준다.
