62 years, male

투석한지 6년 된 환자로 4년전 right radiocephalic fistula로 shunt operation0] 시행되었고, 고혈압, 당뇨와 같은 합병증은 없는 상태였다. 1주전부터 시작된 투석관 기능 저하를 주소로 본원 외래로 내원하였다.

Initial shuntogram 상 total stenotic occlusion된 antecubital vein과 cephalic & basilic veins이 확인되었다(Fig. 1) 투시하에서 더 이상의 outflow vein을 확인할 수 없었으며 초음파상 1mm 크기에 가까운 흔적으로 확인되었다(Fig. 2). 7Fr, 24cm sheath(Accu-Sheath, Sung wonmedical, Korea, Cheongju) 로 폐색된 앞부위까지 전진시킨 후, 투시를 보조수단으로 하며 초음파를 main 유도 기구로 사용하여 real time으로 초음파 probe를 0.035’ J-tip & straight-tip guide wires(Terumo, Tokyo, Japan)와 5 Fr Kumpe catheter(Soft-Vu, AngioDynamics, USA, NY) 바로 위에 위치 시키면서 wire를 전진시켰다(Fig. 3). 시술 중 guide wire 가 혈관 외부로 뚫고 나가며 parenchymal injury를 유발하였다(Fig. 4A, B). 그 후 venous true lumen을 찾 angioplasty를 시행하였다(Fig. 5). 마지막 shuntogram 상 더 이상의 혈관 손상이 없이 재개통된 것을 확인하고 시술을 종료하였다(Fig. 6).

Final shuntogram : relatively good out flow in AV fistula

고식적인 투석혈관의 혈관성형술을 넘어 초음파유도하 혈관성형술을 시행하는 추세이다. 하지만 이는 협착에 의한 투석혈관의 혈관성형술에 국한된 것으로 본 증례와 같이 초음파 유도하에서 폐색된 혈관으로 유도철사의 진입을 통하여 chronic total occlusion 된 venous lesion에 혈관 성형을 시도할 수도 있을 것이다.
