54세/ 남자

Whipples' Surgery후 발생한 황달

췌장두부를 침범한 진행위암으로 Whipples' surgery를 시행받고 4일 후 bilirubin 수치가 수술전 0.4 mg/dL에서 8.8 mg/dL로 상승하였다. 초음파 소견상 간내담도와 총담관이 늘어나 있으며 담낭은 collapse되어 있었다. Post-op 10일 째에 PTBD와 percutaneous cholecystostomy를 시행하였다. Tubogram상에서 CBD가 수술중 결찰에 의해 막혀있고 cystic duct 역시 조영되지 않아 low insertion of cystic duct를 가진 환자에서 cholecystojejunostomy 후 cystic duct와 common bile duct가 모두 결찰된 것으로 생각되었다. Cholecystojejunostomy는 patent하였다 (Fig.1a).

Cholecystostomy를 통하여 8F arterial sheath를 dilator와 함께 insertion하여 GB 쪽에서 가볍게 밀어 보아 GB neck과 common bile duct가 가장 가깝게 위치하는 지점을 찾아서 puncture site를 정하였다. 이후 dilator는 빼고 GB 내강에 남겨두었던 arterial sheath를 통하여 22 guage Chiba needle을 넣고 common hepatic duct를 천자하였다 (Fig. 1b). 0.018 inch guidewire를 tract에 남겨두고 needle을 빼면서 Y-connector로 조영제를 arterial sheath를 통하여 넣어보아 puncture된 tract에 vascular structure가 없는 것을 확인하였다. Tract을 통하여 10F pigtail catheter를 insertion하여 catheter top은 common hepatic duct에 위치시켰다(Fig.1c). 이렇게 해서 만든 cholecystocholedochal connection을 6주간 두어서 mature fibrous tract이 형성되기를 기다렸다. 6주 후에 tract에 safety wire를 남겨두고 catheter를 제거한 다음 조영제를 주입해 보아 leakage가 없는 지를 확인한 후 (Fig.1d), 10mm x 6cm 의 Gianturco type stent를 tract에 위치시켰다(Fig. 1e). 3일 후에 PTBD catheter를 통하여 tubogram을 시행하였을 때 stent lumen이 patent하였으며, leakage 역시 없어 PTBD tube를 제거하였다. 환자는 이후 5개월 동안 jaundice나 cholangitis의 증상이 없었다.

Six weeks later, imaging of the tracts was performed by injecting contrast media through an endhole catheter into the tract. There is a well-formed tract between the gallbladder and the common hepatic duct without leakage (arrow). e. 10 mm diameter, 6 cm long, Gianturco type stent was inserted in the tract. There is a good passage of the contrast media through the stent into the gallbladder and jejunum. Three days later, PTBD tube was removed and the patient is free of symptom for five months.

본 환자는 비정상적으로 low-insertion cystic duct가 있어서 distal CBD와 함께 ligation이 된 것으로 생각되며, 이로 인하여 common hepatic duct와 gall bladder와의 communication이 없어 obstructive jaundice가 초래되었다. Gall bladder 에서 common hepatic duct를 puncture할 때에 hepatic artery나 cystic artery에 손상을 주지 않기 위하여 anterior direction으로 puncture하는 것으로 되어있다. 그 이유는 Calot's triangle (bounded by cystic duct, common bile duct and inferior border of liver) 내에 hepatic artery와 cystic artery가 주행을 하게 되는 데 75%의 환자에서 hepatic artery가 CHD의 뒤쪽으로 지나가기 때문이다. 또한 cystic duct orifice 근처의 CBD를 puncture하게 되는 데, 이는 cystic artery가 GB neck 근처에서 인접하여 있지만 그 이외의 곳에서는 cystic duct와 접하지 않기 때문이다. 한편 needle tract에 대한 pullback study를 함으로써 major vascular damage를 줄일 수 있다. 결론적으로, self-expandable cholecysto-choledochostomy는 malignant disease가 있는 환자에서 non-functioning cholecystojejunostomy로 인한 obstructive jaundice를 해결해주는 유용하고 안전한 시술방법으로 사료된다.
