54세/ 남자

황달이 있어 담석과 담낭염으로 담낭 절제술을 시행받고 병리조직검사에서 담낭암 진단 받음. 그 후 다시 황달이 생겨 모 대학병원에서 ENBD 시행받고 본원으로 ER 통해 입원함. 입원 후 담낭 CT와 초음파를 시행함.

담낭암에 의한 임파절 전이로 생긴 총수담관 폐쇄

응급실에서 시행한 복부 CT에서 췌장 두부 주위에 1-3 cm 크기의 Lymph node가 보이며 이 node에 의해서 distal CBD가 막혀 있으며 retroperitoneum과 liver에 multiple metastasis 소견을 보임. ERCP에서 역시 distal CBD obstruction이 있으며 십이지장에 궤양과 종괴가 있어 조직생검을 시행하고 ERCP후 내시경을 통해 stent를 삽입함(Fig.1). 그 후 계속 황달이 지속되어 stent가 막혀 있는 것을 초음파를 통해 확인하고 endoscopy로 stent 제거를 시도했으나 실패하고 이 후 환자는 패혈증 증세를 보여 PTBD를 시행함(Fig.2). 다시 임상의와 논의 후 방사선학적 접근 방법으로 stent를 제거하기로 함.

PTBD tract을 통하여 7F Peel away sheath를 넣고 먼저 Safety guide wire를 duodenum까지 넣은 후 (Fig.3) J-wire를 두겹으로 peel away sheath를 통하여 넣은 후 wire twisting과 modified snaring technique을 사용하여(Fig.4) stent를 제거함(Fig.5).

The stent was removed successfully along the PTBD tract. We performed another PTBD through the safety guide wire.

Distal CBD obstruction 환자에서 내시경을 통해 stent를 삽입한 경우에 stent가 막힌 경우나 잘못된 위치에 있을 때 내시경을 가지고 제거하지 못하는 경우에 다른 처치 방법이 없었으나 이번의 경우는 PTBD를 시행하고 Peel away sheath를 통해 twisted double guide wire technique과 modified snaring technique를 이용해 stent를 성공적으로 제거한 증례로 이미 발표된 보고를 review해 보면 이러한 증례는 보고된 바 없으며 undeployed vascular stent (1,5)나 vascular foreign body (3,4) ureteric stent (2)의 제거나 교체 시 double-wire technique과 gooseneck snare를 사용한 보고가 있다. 이번 증례는 twisted double guide-wire technique를 이용하여 non-functioning biliary stent의 제거에 적용한 증례이며 이와 유사한 증례가 앞으로 많이 발생할 것으로 생각된다.
