Obstructive jaundice를 주소로 입원하여 CT상 pancreas head portion에 mass가 있고 angiography상 superior mesenteric vein 및 portal invasion으로 수술의 적응증이 되지 않는 pancreas cancer환자로 담도 폐쇄의 완화를 위하여 스텐트 삽입술을 시행하였다.

PTBD를 시행하고 8.5F pigtail Cope-loop drainage catheter를 삽입한 후 2주일 째 길이 70mm, 직경 8mm의 EndoCoil biliary stent(Instent, MN, USA)를 distal CBD에 삽입하였다(Fig.1). 스텐트는 삽입 즉시 최대 직경 8mm로 팽창하였고 시술 당시 통증 외에 부작용은 없었다. 시술 후 8개월 째 obstructive jaundice가 다시 나타났고 이 당시 시행한 단순복부촬영상 스텐트의 이동을 확인할 수 있었다(Fig.2). 환자는 스텐트가 대변으로 빠져나온 사실을 모르고 있었다.

Eight months later, we cannot find EndoCoil stent on simple abdomen by the migration of the stent.

수술의 적응증이 안되는 악성 담도 폐쇄 환자를 대상으로 다양한 종류의 metallic stent - Wallstent, Gianturco-Roesh stent, Strecker stent, Hanaro stent가 사용되고 있다. metallic stent는 plastic prostheses보다 시술 후 부작용이 적으며 스텐트의 patency rate가 높은 것으로 보고 되고 있다. 각각의 metallic stent는 재질 및 제조 방법이 달라 radio-opacity, flexibility, cost, deployment system, radial force, stent shortening면에서 장단점이 있으며 새로운 스텐트가 꾸준히 개발되고 있다. EndoCoil stent는 nickel-titanium 형상기억합금의 flat wire로 coil tube의 구조로 만든 스텐트로 stainless steel보다 radial force가 강하고 탄력성과 기억력이 좋아 더 빨리 팽창하고, wire가 closed loop를 형성하여 tumor ingrowth를 방지하고, 스텐트의 양끝이 부드러워 bile duct, duodenum의 penetration을 예방하고, 스텐트의 한끝을 잡아당겨 제거할 수 있는 장점이 있다. 그러나 시술 시 스텐트의 양끝이 풀리면서 급히 팽창하여 통증과 출혈이 많으며 다른 metallic stent보다 가격이 비싼 단점이 잇으며 스텐트의 표면이 매끈하여 드물게 본 증례와 같이 스텐트의 이동이 보고되고 있다. 최근 본원에서 국내에서 개발되었고, 가격이 싸고, radial force가 좋으며, 중등도의 유연성을 가진 Hanaro stent (Sooho Meditech, Seoul, Korea)를 담도용 스텐트로 사용하고 있다. Hanaro stent 역시 개선점이 많아 꾸준한 개발이 요구되고 있다.
