임상 소견
초기 혈관 촬영상 좌측 제2부 중뇌동맥의 폐쇄소견이 판찰됨 (Fig. 1). Urokinase 300,000unit를 사용한 후에 별다른 변화가 없어 보임(Fig. 2). 제2부 원위부 중뇌동맥에 urokinase 200,000units를 더 사용한 후 제3부 기시부의 혈관에 약간의 경색이 보이나 3부, 4부 중뇌동맥이 잘 관찰 되고 있음(Fig. 3, 4).

시술방법 및 재료
6F Cordis guiding cothter를 이용하여 좌측내경동맥에 위지시 킨 후 micro target infusion catheter를 사용하여 multihole이] thrombus 내에 위치시키게 한 후 urokinase 10만 unit률 normal saline 10cc에 혼합한 후 three way connector를 통하여 1CC lock syringe에 옮긴 후 천천히 주입하고, urokinase 300,000 unit infusion 후에 다시 혈관 조영, 다시 urokinase 200,000 unit infusion 후 조영상에서 좋은 결과를 보였음.
고찰
Urokinase의 혈전용해 효과는 여러 문헌의 보고가 있다. 특히 4시간 이내의 혈전에는 효과가 탁월한 것으로 보여진다. 혈관재개통을 위한 Urokinase의 평균 주입시간은 catheter의 모양, 주입 방법에 따라 많은 차이가 있으나, pulse spray method에 의한 방법이 가장 짧은 시간을 보고하고 있어, 혈관촬영실내에서 모든 시술을 마칠 수 있는 이점이 있다. 혈전 용해술의 합병증으로 원하지 않는 출혈과 원위부 색전증, catheter 주위의 혈전증이 생길 수 있다. 본 증례의 경우 1시간 30분 동안 urokinase infusion을 혈관 촬영실에서 시행하여 혈전을 용해하여 혈관의 재개통을 이룰 수 있었으며 합병증은 생기지 않았다.
