Fogerty catheter, thrombectomy, catheters and catheterization, complication

55세 /남자

3일간의 left calf pain을 주소로 내원. Tenderness와 coldness 동반. 7개월 전 brain tumor로 수술 후 약물 치료와 방사선 치료를 받음.

Fogarty catheter 를 이용한 동맥 혈전 제거술 후 합병증으로 발생한 peroneal arteriovenous fistula

Femoral angiogramol] left superficial femoral artery의 distal portion의 완전 폐색이 관찰되고, 측부혈행에 의해 anterior 및 posterior tibial artery의 근위부가 조영된다(Fig, 1, 2).

Popliteal artery 절개 후, 3F Fogarty catheter를 이용하여 상부(femoral artery side)와 하부(tibial artery side)로 thrombectomy를 시행하였으며, 그 후 환자의 증상이 호전되었다. 퇴원 전 추적 angiogram에서 peroneal arteriovenous fistula가 발견되었다(Fig, 3, 4). Microferret catheter (Cook, Bloomington, USA)s} 0.018 inch Gianturco coil (Cook, Bloomington, USA) 2개를 이용하여 fistula 상, 하 부의 peroneal artery 색전을 시행하였다. 시술 후 angiogram에서 fistula는 관찰되지 않았다(Fig. 5, 6).

1963년, Fogarty에 의해 embolectomy catheter가 소개된 이후, 동맥 혹은 정맥의 thromboembolic disease의 치료에 혈관외과 의사들이 가장 많이 이용하는 기구가 Fogarty balloon catheter이다. 기구가 다투기 쉽고 비교적 안전하여 합병증의 빈도는 낮으나 혈관벽의 손상에 의한 몇몇 심각한 합병중을 초래할 수 있으며 그 빈도는 0.5-0.7%로 보고되어 있다. Fogarty 카테터를 이용한 thrombectomy 후 발생한 peroneal arteriovenous fistula는 지금까지 9예가 보고되어 있으며 이 함병증의 치료에 대해서는 아직 논란이 있으나, Acin 등은 fistula로 인해 distal part의 ischemia가 심해진다고 하여 발견 즉시 수술적인 치료를 시행할 것을 권장하고 있다. 본 증례에서도 fistula에 의한 직접적인 증상은 없었으나 coil을 이용한 중재적 시술로 성공적으로 치료하였다. 이때 주의할 점은 peroneal artery가 ankle level에서 tibial artery와 연결될수 있으므로 fistula회 상부와 하부를 동시에 색전시켜야한다.
