하루 전 갑작스런 우측 옆구리와 우상복부의 통증을 주소로 응급실에 내원

우측신동맥류파열

복부 cry 우측 perinephric space에 많은 양의 혈종이 차 서 하대정맥과 우측신정맥을 앞으로 밀고 있고, 조영제가 perinephric space로 빠져 나간 고여있는 소견이 관찰된다 (Fig. 1). 우측 신동맥조영술 상 우측 주 신동맥 중간 부위에 narrow neck을 가지고 위쪽으로 향하는 약 7mm 크기의 동 맥류 낭이 관찰되고 이 동맥류 낭에서 조영제가 새면서 주위 로 고이는 모습이 보인다. 또한 이 동맥류 낭에서 우측 신장 의 lower pole을 공급하는 신동맥 가지가 기시 하는 것이 보 인다 (Fig. 2).

Diagnostic catheter를 7 F Cyber guiding catheter(Meditech, Waterstone, USA)로 교환하고 Fas tracker 10 미세도관 (Target, Fremont, USA)을 유도카테터를 통하여 coaxial로 진행시켜 동맥류의 색전술에 이용하였다. 카테터의 근위부는 rotating haemostatic valve를 부착하여 heparin solution으로 continuous flushing하여 카테터 사이의 혈전 생성을 예방하였다. 동맥류 낭의 초 선택은 Seeker lite-10 microguidewire (Target, Fremont, USA)로 시도 하였다. Road mapping 의 도움과 torque device# 사용하여 미세유도철사를 동맥류 낭 내로 넣은 후 미세도관을 진행시켜 동맥류 낭 내부에 그 tip이 위치하도록 하였다. Embolotherapy전에 미세도관을 통하여 손으로 주사하면서 aneurysmography를 시행하여 동맥류의 angioarchitecture를 확인한 후 8mmx20cm, 3mm x 8cm, 2mm x 6cm Guglielmi Detachable Coils 3개를 가장 큰 직경의 coil 부터 차례로 미세도관을 통하여 동맥류 낭내부에 deployment 시켰다. Embolotherapy후 유도카테터를 통하여 신동맥 조영술을 시행하였을 때 동맥류낭은 완전히 obliteration 된 것을 확인 할 수 있었고, 더 이상의 조영제의 leakage는 없었다 (Fig. 3 A, B). 그러나 동맥류 낭에서 기시했던 lower pole branch가 같이 폐쇄된 것이 보였다. 추적 CT에서 동맥류 내에 radiopaque GDCs가 보였고, 더 이상의 조영제의 leakage는 없었으며, 우측 신장은 잘 조영 되었고, perinephric hematoma의 양도 줄어 있었다 (Fig. 4).

신 동맥류는 드문 혈관 기형으로 모든 동맥류의 1%, 복부동맥류의 15~20%를 차지하며 신 동맥류 파열의 빈도는 약 5% 정도로 드문 합병증인 것으로 보고 되고 있다. 이런 신동맥류의 치료로서 endovascular treatment는 수술보다 훨씬 덜 침습적이기 때문에 수술을 대치 할 수 있는 시술로서 지난 몇 년간 관심이 높아지고 있다. 하지만 visceral intervention에서 주로 사용하는 standard catheter는 그 stiffness 때문에 distal vascular access를 시도하거나 작은 혈관 또는 얇은 벽을 가지는 동맥류 등을 초선택 할 때 많은 제한이 있다. 그러나 요즈음 주로 neurointerventional radiology에서 사용하고 있는 microcatheter - microguidewire system과 새로운 embolic material의 발전이 이러한 visceral vascular lesion의 초 선택을 가능하게 하였고 1997년에는 Guglielmi Detachable CoilsS 이용하여 8개의 신동맥류를 치료한 증례가 보고 되었다. 본 증례도 그와 유사한 것으로 작은 신 동맥류가 파열되어 perinephric space로 혈종을 형성한 경우 기존의 microcoil등 으로는 parent renal artery의 희생없이 동맥류낭만을 색전시켜 하는 것이 힘들지만 GDCs는 그 high flexibility와 softness, 그리고 controlled detachment가 가능하기 때문에 작은 동맥류나, wide neck을 가지는 동맥류에서도 안전하고 완전하게 동맥류를 폐쇄시킬 수 있어 이와같은 신동맥류 치료에 매우 유용하다.
