간이식술 후 발생한 상복부 통증 및 상부 위장관 출혈.

Celiac angiography에서 간동맥은 정상이었으나 비장동맥의 기시부에 약 12mm크기의 가성동맥류(pseudoaneurysm)가 보였다(Fig. 1A). 상장간막 동맥을 통한 간접 문맥 조영에서 문맥의 내경감소가 의심되어 좌문맥을 직접 천자하여 얻은 직접 문맥조영술에서 주문맥에서 좌문맥 분지에 걸친 약 3cm길이의 내경 감소가 확인되었으며 주문맥압과 좌문맥압이 각각 28mm Hg와 12mmHg로 16mmHg의 심한 압력 차이를 보였다(Fig. 2A).

좌문맥을 직접 천자하여 7-Fr sheath를 위치시키고 이를 통하여 직경 10mm, 길이 69mm의 Wallstent (Schneider, Switzerland)를 협착부위에 삽입하였다. 스탠트 삽입 후의 문맥조영에서 협착 없이 순탄한 혈류를 보였으며 주문맥과 좌문맥의 압력이 20mmHg와 23mmHg로 압력 차이가 16mmHg에서 3mmHg로 현저한 감소를 보였다(Fig. 2B). 2일 후에 비장동맥의 가성 동맥류의 기시부에서 원위부까지 8개의 micro coil (3mm×2cm)을 사용하여 색전술을 시행하였다. 색전술 후 동맥류와 비장 동맥의 기시부위가 보이지 않으나 비장은 측부 순환을 통하여 혈류공급이 유지되었고(Fig.1B) 추적 CT에서도 비장경색은 보이지 않았다.

간이식술 후에는 infection, rejection, renal insufficiency, neurologic complication, hemorrhage 등의 합병증이 발생할 수 있으며 이외에도 혈관과 관련되어서는 간동맥이나 문맥의 혈전증이나 연결부위의 협착, 가성 동맥류 등이 생길 수 있다. 또한 담도의 협착이나 bile leakage등이 생길 수도 있다(1,2). 문맥 협착의 발생율 약 3% 이하로 보고 되고 있으며 풍선 확장술이나 스탠트를 이용한 치료가 보고되고 있다(3). 문맥의 협착은 수개월부터 수년 후에도 생길 수 있으며 문맥압의 상승으로 인하여 splenomegaly, ascites, variceal bleeding, anemia등의 증상이 나타날 수 있다. 본 증례에서는 문맥압의 상승으로 인한 variceal bleeding이 생겼던 경우로 보이며 시술 후 출혈은 멈추었다. 드물게 수술 전 조직검사 등에 의한 간혈관의 손상으로 pseudoaneurysm이나 arteriovenous fistula가 생길 수 있으며 hemobilia, hemoperitoneum, gastrointestinal bleeding의 원인이 될 수 있다. 본 예와 같이 비장 동맥 기시부에 pseudoaneurysm이 발생한 경우는 매우 드물다. 비장의 혈류 공급은 비장 동맥 기시부의 폐쇄시 비장 주변의 여러 측부 순환로가 쉽게 발달이 되기 때문에 비교적 안전하게 비장동맥의 기시부의 색전술을 시행할 수 있다.
