Fistula, uretera/ Fistula, therapeutic blockade

57세/여자

자궁경부암으로 수술 후 방사선치료를 받은 과거력이 있음. 하복부로 소변의 누출이 있어 좌측 신루술 후 4개월이 경과하였으나 소변의 누출이 계속됨.

요관누공

신루조영술에서 원위부 요관의 완전폐쇄와 폐쇄부위 상부에서 누공을 통한 조영제의 누출이 보인다 (Fig. 1).

신루설치술 경로를 통하여 5F Cobra catheter(Cook, Bloomington, U.S.A.)를 이용하여 10mm×5cm, 8mm×4cm Gianturco coil(Cook)을 요관협착 부위에서 윗 방향으로 길게 늘려서 넣는다. 이후 10개의 5mm×5cm Gianturco coil과 Gelfoam(Upjohn, Kalamazoo, MI)을 3mm×3mm 크기로 잘라서 폐쇄부위에서 윗 방향으로 교대로 밀어 넣는다. 마지막 신루조영술에서 더 이상의 조영제 누출은 보이지 않는다 (Fig.2). 2개월 후 신루술 카테터를 교환하기 위하여 내원하여 촬영한 신루조영술에서 조영제의 누출은 없었다 (Fig. 3).

요관누공의 외과적 치료는 매우 어려운데 특히 악성종양에 의한 경우는 재발이 흔한 것이 특징이다. 요관누공에 의한 소변의 누출은 환자의 정신적인 문제와 관련이 있기 때문에 life expectancy가 짧아도 치료를 원칙으로 한다. 요관누공이 작은 경우는 external drain만으로 충분하지만큰 경우에는 충분치가 않다. 경피적요관폐쇄술은 여러 가지 방법이 제시되고 있다. Schild(1)는 silicone-filled detachable balloon과 Glue를 이용하였고, Papani-colaou(2)는 또다른 경로를 만들어 nondetachable balloon occlusion catheter를 이용하였다. Farrell(3)은 후복막강으로 접근하여 stainless steel clip으로 요관을 직접 결찰하였다. Coil과 Gelfoam을 이용한 요관폐쇄술은 Gaylord(4)에 의해 처음 보고되었다. 이 방법은 쉽고 합병증 없이 시술할 수 있으며 요관의 폐쇄는 coil에 의한 uroepithelial hyperplasia에 의한 협착에 의한다. 나중에 coil이 이동되는 경우가 있기 때문에 적절한 크기의 coil을 선택하여야 하며 ureteroiliac artery fistula가 생기는 경우도 있으므로 장골동맥과 요관이 교차하는 부위보다는 하방에 coil이 위치하도록 하는 것이 중요하다(5).
