41세/남자

평소 건강하였으나 2주전부터 좌측 하지의 종창과 통증이 발생함.

May-Thurner syndrome

좌측 하지의 상행 정맥조영술에서 총대퇴정맥과 장골정맥이 조영되지 않고 주변에 부행혈관이 조영됨 (Fig. 1). May-Thurner 증후군을 의심하고 확진을 위해 시행한 CT 정맥조영술에서 좌측 장골정맥이 우측 장골동맥에 의해 압박 받고 있으며 그 원위부의 장골정맥과 하대정맥에 혈전이 있어 (Fig. 2A, 2B) May-Thurner 증후군으로 진단함. 경정맥을 통하여 하대정맥 필터를 설치하고 내과적인 치료를 하며 환자에게 중재술을 권유했으나 거절하고 진단 4주 후 증상이 악화되어 내원하여 중재술을 하기로 함.

환자를 복와위로 하여 좌측 슬와정맥을 천자한 후 6 French sheath를 삽입하고 조영하였을 때 표재 대퇴정맥에 혈전이 있어(Fig. 3) Mewissen infusion catheter (Boston Scientific/Medi-tech, Watertown, MA)와 Katzen Infusion Wire (Boston Scientific/Medi-tech, Watertown, MA)를 coaxial로 위치시키고 각각에 유로키나제를 2,000unit/min의 속도로 주입하기 시작하여 폐색이 있는 장골정맥까지 올리면서 주입하였다. 30시간에 걸쳐 총 350만 unit의 유로키나제 주입 후 혈관 조영에서 혈류는 개통되었으나 장골정맥에 혈전이 일부 남아 있었다(Fig. 4). 장골정맥의 폐색부위로 유도철사를 통과시킨 후 10mm 풍선으로 혈관성형술을 시행하고(Fig. 5) 직경 10mm, 길이 8cm의 Wallstent를 설치하였고 슬와정맥에서 조영제를 주입하여 시행한 정맥조영에서 하대정맥으로의 혈류가 정상화되었다 (Fig. 6A, 6B). 9개월 후에 시행한 CT 정맥조영술에서 혈전의 형성 없이 혈류가 유지되고 있었다 (Fig. 7).

May-Thurner 증후군은 좌측 총장골정맥이 척추 앞에서 우측 총장골동맥에 의해 눌리고 동맥의 맥동이 자극이 되어 정맥 벽의 내막과 중막이 내피세포가 덮힌 결체조직으로 바뀌면서 정맥내강으로 돌출하는 돌기(spur)에 의해 정맥내강을 좁혀 혈류 장애를 일으켜 장골정맥이 혈전으로 막히는 질환이다. 건강한 성인에서 특별한 이유없이 발병한 좌측 장골정맥과 대퇴정맥에 심부정맥혈전증이 있을 때 의심해 봐야 한다. 상행정맥조영술 만으로는 확진이 어렵고 CT 정맥조영술을 하여 좌측 총장골정맥이 우측 총장골동맥에 의해 눌리는 것을 확인하여야 한다. 치료는 본 증례와 같이 슬와정맥을 천자하여 카테타를 이용한 혈전용해술을 시행하고 우측 총장골동맥에 의해 좌측 총장골정맥이 눌리는 부위에 스텐트를 삽입한다. 이 시술의 초기 기술적 성공률은 87%이고 1년 개통율은 79%이며 85%의 환자에서 임상적으로 증상의 호전이 있다. 장골정맥의 혈전증에 의한 폐색전증의 빈도는 높으나 장골정맥압박 증후군에 의한 혈전증의 경우는 돌기가 혈전을 막아주는 역할을 하기 때문에 드물다고 한다. 따라서 하대정맥 필터의 삽입이 필요 없을 것으로 생각되나 이 증례에서는 하대정맥으로 돌출하는 비교적 큰 혈전이 있어 하대정맥 필터를 삽입하였다. 이 환자는 9개월 후의 추적검사에서 장골정맥이 개통되어 있었고 증상 없이 정상적인 생활이 가능했다.
