3일간의 간헐적인 복통으로 보존적 치료를 받았으나 5주 후 증상이 다시 발생하여 내원하였다.

복부전산화촬영상 상장간막동맥의 근위부에 저음영의 혈전이 관찰되며 이로 인한 상장간막동맥의 내경 감소가 관찰된다 (Fig. 1). 복부대동맥 혈관조영술상 상장간막동맥의 근위부에서 시작되는 내막판(intimal flap)이 보이며 가강(flase lumen)에 의한 진강(true lumen)의 압박이 관찰된다 (Fig. 2, 3).

우측 대퇴 동맥을 천자하여 8F sheath를 삽입하고 8F renal guiding sheath를 상장간막동맥 기시부까지 삽입한 후 5F catheter를 이용하여 상장간막동맥의 진강을 초선택한 후, 8mm×14mm Corinthian 스텐트를 협착 부위에 삽입하였다(Fig. 4). 이후 시행한 상장간막동맥 혈관조영술상 내경 감소가 개선되었으며 스텐트 삽입 전에 복부대동맥과 상장간막동맥 원위부의 압력차는 30mmHg로 측정되었으나 스텐트 삽입 후 압력차는 사라졌다.

대동맥박리 없이 발생한 상장간막동맥에서의 자발성, 단독성박리는 보고된 증례가 적은 드문 병리로 빠른 진단과 적극적인 수술적 치료를 요하며 사망률이 높은 것으로 보고되어 있다. 그 원인은 아직 잘 알려져 있지 않으나 외상에 의한 내막, 중막의 손상 또는 마르팡 증후군(Marfan syndrome), 섬유근성증식증(fibromuscular dysplasia)과 같은 질환에 따른 중막변성 등으로 설명하고 있다. 증상은 수주에서 수개월간에 걸친 intestinal angina로부터 장괴사에 이르는 급성 장폐색에 이르기까지 다양하다. 예전에는 장간막 혈관의 협착 또는 폐색시에 우회로이식조성술(bypass graft) 등과 같은 수술적 치료만을 고려하였으나 최근에는 스텐트삽입술이 혈관성형술과 함께 일차적인 치료법으로 많이 이용되고 있으며, 자발성 상장간막동맥박리에 대한 경피적 혈관내 스텐트 삽입술은 최소한의 침습적 치료로서 특히 수술전후의 합병증이 높을 것으로 예상되는 환자에서 안전하고 유용하게 이용될 수 있다.
