62세/남자

환자는 원인불명의 열로 치료 받던 중, 우측 하지의 claudication을 호소하였다.

내원 당시 CT 에서 aortoiliac junction 직상부의 대동맥에 약 3.3cm 크기의 동맥류가 형성되어 있었고 대동맥벽이 비교적 두껍게 조영증강 되는 것이 보였다(Fig. 1). 우총장골동맥에 약 80%정도의 stenosis가 있어 혈관조영술을 시행하였다. 우총장골동맥을 통하여 approach 하였으며, 5-F Pigtail catheter (Cook, Bloominton, IL)을 이용하여 대동맥조영술을 시행하였고, 혈관조영술에서 우총장골동맥의 협착이 약 70% 정도로 관찰되었다. 이후 8mm balloon을 이용하여 혈관성형술을 하고, Easy Wallstent(Schneider, Bulach, Switzland) 8×95mm를 설치하였으나, 협착부위가 완전히 cover되지 않아 아래 부분에 10×65mm의 Easy Wall stent를 하나 더 설치 하였고, 협착부위는 완전히 cover 된 것으로 확인하였다. 혈관조영술에서 좌장골동맥에도 협착이 있었지만, 2개월 후에 추적검사와 혈관확장술을 시행하기로 하였다. 시술한 지 2개월 후부터 하복부에 pulsating mass가 만져졌고 CT-혈관조영술에서 복부대동맥류의 확장이 확인되었다(Fig. 2). 복부대동맥류가 최대지름 약 6cm까지 커져서 rapid growth한 것을 확인하고 파열의 위험이 높아 수술(aortobiiliac bypass graft)을 시행 받았다. 수술에서 aneurysm은 그 최대지름이 약 7cm로 측정되었으며, stent가 들어있는 우총장골동맥은 주위 복막과 심한 유착이 있어, 이를 박리해 내지 못하였고, 삽입된 두개의 stent 중 하나만 제거가 가능했다. Aorta clamp 후에 inverted Y-shape의 Hemashield 16×8mm를 infrarenal aorta에서부터 우측은 우총장골동맥에, 좌측은 외장골동맥에서 연결하였다.수술 후 1개월 뒤의 CT 혈관조영술에서 혈류는 잘 유지되어 있었다(Fig. 3).

Aortoiliac artery의 동맥성형술 이후에 생기는 합병증은 약 8%에서 보고되고 있는 데, 이들은 주로 성형부위에 발생하는 dissection, acute thrombosis, distal embolism, vasospasm 등에 의한 급성폐색과 천자부위에서 발생되는 hematoma, pseudoaneurysm 등이다. 문헌에 의하면 aortoiliac stent 삽입 후의 직접적인 합병증으로 가성동맥류 등이 발생할 수 있다고 알려져 있다. 그러나 본 증례와 같이 stent insertion과 대동맥의 동맥류가 갑자기 커지는 것은 잘 알려져 있지 않다. 이러한 합병증이 생기는 이유로 stent insertion 당시 catheter나 stiff guide wire등에 의한 aortic aneurysm의 injury를 추정해 볼 수 있지만 본 증례에서는 환자가 처음 내원할 당시에 발열이 있었고 CT 소견에서 대동맥류의 벽이 두껍게 조영증강되는 등 감염성 대동맥류의 가능성을 배제하기 어렵다고 판단된다.
