63세/여자

고혈압의 과거력 있으나 치료하지 않고 지내던 환자로 약 1개월 간의 양측 어깨의 통증, 약 보름 간의 복통이 있었으며 내원 3일 전부터 양측 어깨의 통증이 심해져 내원함

가강 파열을 동반한 대동맥 박리증(DeBakey type III)

조영증강 CT에서 하행흉부대동맥에서부터 복부대동맥에 걸친 대동맥 박리가 관찰된다. 가강(flase lumen) 내에서 조영증강을 보이는 혈류가 관찰되며 일부에서 혈전이 형성되어 있다 (Fig. 1A). 횡격막 부근의 대동맥에서 진강(true lumen)은 가강에 의해 심하게 압박(compression)되어 있으며 현저히 확장된 가강 주변으로 액체 저류가 관찰되어 가강의 파열이 의심된다 (Fig. 1B). 대동맥 조영술상 일반적인 대동맥 박리증의 경우와 다르게 대동맥궁의 소만곡(lesser curvature)쪽에서 entry tear가 관찰된다 (Fig. 1C). 좌신동맥이 가강에서 기시하며 이 부위에서 reentry tear가 관찰된다 (Fig. 1D).

우총장골동맥을 통하여 삽입한 12 F sheath를 하행 흉부 대동맥에 위치시키고 좌쇄골하동맥 기시부하방에 위치한 entry tear를 cover하도록 stent graft를 deploy 하였다. 시술 후 entry tear가 stent-graft 에 의해 막히고 진강의 혈류는 잘 유지되어 있다 (Fig. 2A).

대동맥 박리증에서 사망에 이르는 두 가지 중요한 합병증은 가강의 압력이 높아지면서 발생하는 가강의 파열과 분지 혈관의 폐쇄로 인한 합병증으로 적극적인 치료를 요한다. 경피적 stent-graft 삽입술은 수술과 비교하여 시술자체와 관련된 합병증이 적을 뿐 아니라 대마비(paraplegia) 등의 신경학적 합병증이 적고 재원기간을 줄일 수 있어 최근에는 고식적인 수술법 이외에 경피적 stent-graft 삽입술의 이용이 증가하고 있다. 경피적 stent-graft 삽입술은 entry tear를 막아 가강 내의 혈전 형성을 유도하며 가강 내의 혈전은 시간이 지나면서 점차 흡수된다. 본 증례와 같이 하행 흉부대동맥에 위치한 entry tear를 stent-graft로 막은 경우 추적검사 상에서 흉부의 가강은 혈전에 의해 소실되지만 복부의 가강에서는 여전히 혈류가 남아 있는 것을 볼 수 있는데 이는 복부 대동맥이나 장골동맥에 다수의 reentry site가 있기 때문으로 설명할 수 있다. 본 증례는 가강의 파열이 동반된 대동맥 박리증을 경피적 스텐트 그라프트 삽입술로 치료한 경우로 추적검사에서 대동맥 주변의 혈종이 흡수되고 가강 내에 혈전이 형성되었으며 진강의 혈류가 회복되었다 (Fig. 2B and 2C).
